Program Ad Sponsor Letter & Agreement: A letter to be given to prospective sponsors to encourage and thank them for participating in the 2008 Harrison H. S. Soccer Program Ad campaign and the advertising agreement to be filled out by you and a sponsor to go into the 2008 Harrison H. S. Soccer Program Ad. Physical Form: The Cobb County School District 2006-2007 School Year Policy & Planning Division ATHLETIC PARTICIPATION, INSURANCE, AND CONSENT FORM. If you've already had a physical and submitted the form for another sport this school year, then you DO NOT need to submit this form again for the soccer season. Photo Permission Form: The Cobb County School District's Permission to Display Student Photograph on a school related Internet website. No game photos of a Hoya soccer player will be posted unless a Photo Release From has been turned in to Diane Ritter, the webmaster. These forms will be filed with Mr. Stafford in the High School.
